COVID Course Policy
Prior to attending an in-person course, all attendees who have not been fully vaccinated for COVID-19 must be tested within 4 days of the course they are registered for. Attendees must email a copy or confirmation of their negative test result to email@example.com.
If you have been vaccinated you are not required to take a test. You must provide us proof with a copy of your COVID-19 vaccine dates. This can be emailed to us at firstname.lastname@example.org.
All attendees are required to wear a face mask at all times. Attendees must put on a mask or face covering prior to entering building and continue to wear a mask or face covering throughout the entire course.
Course Cancellation & Refund Policy (Updated April 2018)
If a cancellation is received at least one week prior to the course date, a credit towards another course will be issued. If a cancellation is received within 2-6 days of the course date, a 50% credit towards another course will be issued.
No credit will be issued if a cancellation is received the day prior to or day of the course. Course credits are valid for 12 months after the cancellation date.
Continuing Education Credit
Regardless of the registration fee paid, an attendee must attend the course for the hours it is scheduled, in order to receive the continuing education credit, with one credit equaling one hour.
Per the ADA CERP Standards, should an attendee arrive more than 15 minutes after the course has begun or leave 15 minutes prior to the course ending, the attendee’s course credits will be adjusted accordingly and a new continuing education certificate will be mailed the following week.
To register for the Dental Radiology program, please download the application, fill it out and email the application along with the appropriate documentation to email@example.com. If you would like to visit us in person, you must call beforehand to schedule an appointment. Applicants with diplomas from foreign countries may require extra processing time, call or email the office for more information.
Photography and Recording Notice
Photography, video recording, and audio recording may occur at any NJHPDI event. By attending, you consent to photography, audio recording, video recording and their release, publication, exhibition, or reproduction for any purpose(s) that NJHPDI deems fit. You release NJHPDI and each and all persons involved from any liability connected.
Persons will not be identified using full names or personal information without approval from the recorded subject. A person attending a NJHPDI event who does not wish to have their likeness recorded for distribution should contact NJHPDI at firstname.lastname@example.org in writing of his/her intentions and include a photograph. NJHPDI will use the photo for identification purposes and will hold it in confidence.

NJHPDI reserves the right to cancel or change any courses, instructors, dates and times. The fee charged is for tuition. All food served is complimentary. Please note that photography, videography, and the use of any other recording devices by attendees is strictly prohibited during courses.

In order to receive a resident discount, please call or email the office to register. No changes or refunds will be given if registration is not done according to this policy.
